Contact your county Intellectual Disabilities office, or if you have a Supports Coordinator, skip to Step 2.
Contact your county and request an intake to assess your needs and level of supports.
Arrange a meeting with your Supports Coordinator.
If you're looking for a home that has supports around the clock, you will need federal/state Consolidated Waiver funding. If you're looking for Supported Living or Respite, you may be able to use Consolidated, Community Living, or PFDS federal/state Waiver.
Arrange a home tour.
